Hello,
I know by now there have been hundreds of post in reference to this issue. I have searched this forum and many others looking for a solution.
Problem : http://i.gyazo.com/5ce0a852723cc0dbe54b821877579a0a.png (http://gyazo.com/5ce0a852723cc0dbe54b821877579a0a)
I have tried the following solutions:
Uninstall and re-install game through steam
verify cache integrity
updated every driver on my computer
changed video settings
ran as admin
ran in compatibility mode
many more.......
Yet despite the troubleshooting I have had no luck resolving the issue.
Computer Specs are as follows:

Cpu: Intel Core i5-4670k@3.40Ghz
Ram: 8.00GB Dual-Channel DDR3 @ 798MHz
Mobo: Gigabyte Technology Co., Ltd. Z87X-UD4H-CF
Graphics:
Onboard: Intel HD Graphics 4600
GPU: ASUStek 2047MB NVIDIA GeForce GTX 650 Ti
HDD: 931GB Hitachi HTS541010A9E680
I will also include the dxdiag and the most current crash .dmp.
Thanks in advance for any responses. If you need more information let me know.

Can you try to remove your Nvidia PhysX software and then reinstall it again using the installer from this link?
http://www.nvidia.com/object/physx-9.12.0613-driver.html
It is important that any existing installation is removed first before reinstalling.

Please try installing the Visual C++ 2005 libraries from here: http://www.microsoft.com/en-us/download/details.aspx?id=26347
Hope that helps

Tried both solutions. Neither worked.

Gave up and restored my computer. Works fine afterwards. Thanks for the suggestions anyways.
