Hello,
I registered my Ship Simulator Extreme license key half an hour ago, I restarted the game many times but when I open up multiplayer it tells me I still have not registered??? What do???
Bought the download version on the ship simulator website

In order to access the Ship Simulator Extremes Multiplayer, the License Key have to be the same on the Ship Simulator Forum and in the game. If they are both different, it won't allow you access to Multiplayer. This is for Non-Steam Versions. Steam Versions, I think the Key has to be the same on Steam, the Forum, and the Game. And Welcome to the Forum.

STEAM gives you two keys - so just register both...

Copy and paste from the website, also it tells me I am a registered Extremes user on the fourms

I think you have already registered your "license key" of SSE. This is evident because the word "Extremes" is indicated below your Forum name.
